Participation Requirements
REQUIRED HEALTH PAPERWORK FOR INTERSCHOLASTIC ATHLETICS
Regulations of the Commissioner of Education, Part 135.4 (7)(h).
All students must have a valid physical on file at the start of each sports season. A “valid physical” is defined as having been completed within one year of the month in which the sport starts. The summer is the ideal time to have physicals completed and submitted to the school nurse, as a physical on or after August 1st would satisfy the physical examination requirement for the entire school year.

FamilyID gives you the option to upload your students physical directly to the website. The registration form covers all of the questions on the health interval form that is required by New York State to be cleared for sports. NOTE: a paper copy of the health interval history form is no longer required!
The registration form is to be completed no sooner than 31 days of the start of each season. We will open up registration for each season on familyID. Students must satisfy both requirements (current physical and completed familyID registration) in order to participate on the first day of practice.
